Soyuz 1
Soyuz 1 was the first crewed flight of the Soyuz spacecraft. Vladimir Komarov tested the new vehicle in orbit, but solar-array, attitude-control, and other failures forced an early return. The main parachute failed and the reserve became entangled, destroying the descent module and killing Komarov on impact.
