M-3C-3 attempted to launch Japan's first CORSA X-ray astronomy satellite from Kagoshima Space Center on 4 February 1976. A control-system failure caused the second stage to depart from its planned trajectory, and the integrated upper stages and payload were lost over the Pacific.
