VLS-1 V02 (SACI-2)
VLS-1 prototype V02 attempted to launch the SACI-2 scientific satellite from Alcântara Launch Center during Operação Almenara on 11 December 1999. The first stage operated correctly, but flame penetration near the top of the second-stage propellant block caused an explosion shortly after second-stage ignition.
